Horizon Family Dentistry will notify you of your upcoming appointment two weeks before the scheduled appointment date via email, text, or telephone. We will re-confirm your appointment two days before your visit. We want to ensure you will be in attendance at your reserved appointment time.

Cancellations & Reschedules

We’ve kept essential steps in place to guarantee ease and reassurance throughout your appointments. In our office, we know life can throw curveballs that may delay your arrival for a scheduled visit. To best help all our patients and meet their needs well, we ask for at least two business days’ notice if you need to change or cancel an appointment. This helps avoid missed appointment fees. If you’re running late, we may not have enough time to start or finish your planned treatment. We might need to reschedule in these cases to ensure you get the best care possible. Essential Info for New Patients

For your first visit, make sure your dental insurance details are up-to-date. This helps us process claims quickly and correctly. Tell us if you need to update anything immediately, as we may need to change it based on your provider. We offer electronic or paper filing and can help with either.

If you have insurance, please provide us with your policy details. This includes the name, policy number (Group), certificate number (ID), and division number (if any). Also, tell us the anniversary date, calendar year/policy year, and yearly maximum benefit per patient/fee guide info. This helps you get the most from your benefits.

Extensive dental work like crowns, bridges, or dentures should only be done by licensed dentists.

Insurance Policy

Insured patients are encouraged to provide us the following information about their insurance policy:

  • Name of insurance carrier.
  • Policy number (Group), certificate number (ID), and division number (if any).
  • The anniversary date, Calendar year or policy year.
  • The annual maximum benefit per patient per year.
  • The annual fee guide.
  • Percentage of coverage allowed for diagnostic, preventative, restorative, endodontic, periodontal services and all major treatment such as crowns, bridges or dentures.

Financial Policies

Our clinic typically asks for payment at the time of service. Please talk to our office manager before booking if this isn’t possible. We take Visa, MasterCard, Interac Direct Payment (IDP), and cash for your ease.


Those without health insurance must pay in full when making an appointment. We’ll give you a detailed account statement to review all treatments received, ensuring clarity.



We offer payment plans for extensive treatments. Please set up a meeting with our Treatment Coordinator to discuss options and estimates. We aim to make payment straightforward for all patients, regardless of their insurance status or treatment needs.
